It can take a while to get used to CPAP. Don't be scared to change your own pressure. I had to start at 5 to 7cm, and then slowly move up the pressure as I got comfortable with CPAP. I suggest a small range if you are on Auto and you have to find the perfect pressure for you. Within a few months I found that I slept best on CPAP mode with a set pressure of 13.

I’ve been using it for years and it’s great. Now I also have a module you plug to the right of device for telemetry over mobile network.

I found turning off the softpap completely eliminated all Flow limitations, I have quite a narrow range (14 - 16), most nights I doesn’t hit the 16, I usually start at 10 for 5 mins to help me adjust and usually sleep before 14 kicks in. (I certainly don’t feel it)
This machine is a life changer, can not recommend it enough. I may widen the range as an experiment, buts it’s already performing very well.

Thanks for the great insights mate! sounds like you're pretty much running the device in CPAP mode and have most of your bases covered at that pressure of 14cm. No need to change if it's working well for you. Cheers

I did a sleep test a couple of years ago. First, I felt like I was on an assembly line and the whole thing was designed to shove me into a CPAP machine. The first part of the test was with no CPAP and I had a difficult time getting to sleep. Just about the time I felt like I was sleeping real well, they came in and woke me up. They hooked me up to this machine and it felt like it was going to blow the top off my heard. After the test, they gave me a survey to fill out with my thoughts on the test and I wrote that down and how much I hated it. I could hear the techs in the other room reading my survey and laughing about it. Well, I don't have a CPAP machine and I don't think I will get one.
Great to know! I've been using a Prisma 20A for about 3 weeks and have been trying to dial in the pressure settings. I'm going to give it some room to do it's thing tonight and see how it goes. 👍

No it's not. i'd say 99% of viewers don't know about this brand. It's not even sold in the U.S. I have over 1000 RUclips videos and maybe 5 of them are on this brand. It's also not 'Click Bait' when i'm holding the very device in my arms in the thumbnail
How does Lowenstein algorythm determine the tine spent in each quarter since each night dleep has different durations?

The quartiles in the Lowenstein algorithm aren’t based on time or percentages of your total sleep duration-they’re based on pressure range.
When you set a minimum and maximum pressure (say, 6-12 cmH₂O), the machine splits that pressure range into four equal sections, or “quartiles.” Each quartile represents a zone within the pressure range, not a part of the night.
So for example:
Quartile 1 = 6-7.5 cm
Quartile 2 = 7.5-9 cm
Quartile 3 = 9-10.5 cm
Quartile 4 = 10.5-12 cm
As you breathe through the night, the machine constantly monitors your events and determines which quartile the current pressure is in, and then applies the pressure response based on that zone.
So it's not tied to time-it's tied to where your pressure is sitting within the range. That’s what makes it so dynamic and adaptive to each night’s needs! 😴💨

Question for you Nick, any chance you can do a video on the Lowenstein Auto Cpap range and the differences other than the blower in the Prisma 20A model. @@CPAPReviews Didn't think I had missed any of your videos but maybe it had slipped my mind, my worry re the 20A is when we have power outages which are not unusual in this neck of the woods (Victorian Central Highlands) is powering the machine, are those adapters you showed in that other video commercially available otherwise the Prisma SMART might be the better option for me given I'm considering updating my Dreamstation, I also have a Resmed S10 Auto but I don't like the bloody thing compared to the Dreamstation regards comfort and results.
Oi from Poland! A month ago I got meself a 20A and an AirFit F40 after using an Airsense 10 Autoset with a classic F20 mask on my sleep test for a couple of days in early Feb.
Picked the F40 as the F20 hurt my nose bridge. Let's just say the mask seems like a good product, but I will continue exploring. I'm eyeing a Vitera.
As for the machine... I won't compare, as I don't have enough experience, but I will say this: I turned off dynamic mode as I felt like it told me how I'm supposed to breathe. It didn't work with me. It forced me into a constant rhythm. Standard mode didn't do that. BTW I'm considering a UPPP surgery...
Anyway, I don't want to make this post a wall of text, so I'll finish by saying it's been tough with the CPAP so far! I am keeping my hopes up, though.

Thanks for sharing your experience with the devices-CPAP can definitely be a challenge, so I completely understand where you're coming from.
I'm not a big fan of the UPPP surgery, to be honest. I've had in-depth chats with quite a few people who’ve gone through it, and while there can be some early improvements after a tough recovery, the snoring and poor breathing often tend to come back over time.

The whole concept behind auto CPAP is to dynamically adjust the pressure throughout the night-raising it when events are detected and lowering it during stable breathing. The goal is to average out the pressure so you’re not stuck with a consistently high fixed setting all night.
By delivering just enough pressure when needed and backing off when it's not, auto CPAPs aim to keep therapy effective and more comfortable. In many cases, this results in a lower overall pressure compared to fixed settings, which can reduce side effects like dryness, bloating, or mask leaks.

@@CPAPReviews It's a class II (2) medical device. The FDA has to approve it. Maybe Lowenstein has not submitted it for approval.
Resmed has a virtual monopoly in the US. Phillips no longer sells CPAPS in the US, Fisher and Paykel now out, Luma is still in but small. Now is the time for Löwenstein-Jensen to get into US Market.

I went and got tested and my AHI was 65.5 I’ve been watching your videos to get some knowledge and info before I get set up. I have an appointment Friday to get set up with a machine but I have no idea which one to get or if they will set me up with the whatever machine works. My benifits cover most of it so just wondering if you got recommendations on what I should stay away from or which ones I should try to get if they are available. Thanks
Lowenstein Prisma 20A, I just bought one 2 days ago. Had Philips Resporonic initially then for 16 yrs on various ResMed models. Time for a change, I figured.
Everyone is different. I like the Prisma flexibility to function as an APAP or as a fixed pressure CPAP. It’s the Algorithm I’m most fascinated with. Nicko have quite a few video on this.

I'm trialing this machine now. My min is set to 4, median is 6 and 95 percentile is showing 11. What would be a good range?
Since your pressure requirements are only mild-mod i'd probably just leave the range 4-20cm if you're comfortable breathing at the low end and you don't feel like you're suffocating.

In an older video, you compared the algorithms of different cpap machines, can't remember if lowenstien was on there but it would be dandy if you made an updated video on that which included the two lowenstien modes

Wow, really dedatiled information about the algoritm, where do you find it? Is there similar information about how Resmed 11 works? it would be interesting to compare.
Just curious if you felt any differences from Prisma SMART (Plus or Max, the white one with german blwoer) vs Prisma 20A that has a different Blower. (From my experience they work the same, and feel the same, even if they are different inside). Also I do not know how many people use a heated tube (Prisma 20A has an integrated connection for the heated tube, I never felt the need to use it, But I do not use it with water, maybe I should try it a few nights).
Also 20A has an extra setting for SoftPap level 3 (works almous as a Bipap in this setting)
Thank you! You find it right here in the video lol. I've unpacked a few of the ResMed algorithms over the years but not in the same detail. Some patients feel that SoftPAP is different between the two devices even level 1 and 2. I haven't looked into it much though

I have a prismaLAB which is CPAP GOD MODE but most people would have the Prisma 20A (Darth Vader). Apparently the auto algorithm is also the same for the Prisma Smart (Stormtrooper). Cheers
It ignores flow limitatíons? This explains why my flow limitaton numbers are always high.
In standard mode after quartile one yes. I think this is strategic though to balance comfort.
Does Löwenstein BiLevel and ASV have the same Smart algorithm?

This all dandy, but they're not available from any DME or online store in the US that I'm aware of. Question is why?
Needs to be approved by the FDA. It's a class 2 medical device. Lowenstein has to submit it for approval. It will be pricey with Diaper Don's tariffs.

Do you think CPAP machine's learning Artificial Intelligence will make CPAP treatment better? Instead of a set parameters, the machine will learn your sleep breathing patterns and provides breaths and pressure accordingly? Kind of like a Sleep Apnea Therapist in a machine.

To be honest, I don't think it will improve pressure delivery itself. That part of the therapy likely won't change. However, it can definitely help in other areas of treatment-things like comfort, tracking, or overall effectiveness depending on the context.

Hello friend with you and your sleep doctor work on shooting to the lowest number you can.
Like me if I didn’t use my machine I would have 60 events an hour but by working with my sleep doctor and staying busy to lose weight I have gotten down to 2 events an hour
And you should be getting it pretty much straight away because therapy starts pretty much straight away.
If you are 49 ON a machine get in touch with your sleep doc as you may need asv or bilevel. If haven’t used machine yet, you prob be under 5 treated on a machine,
The $65,000 question is how does this outstanding German/Swiss engineering compare to the RESMED’s top o’the line, ASV? Any opinions you can share?
I've used both. This leaves it for dead.
Great question, they’re a bit different
ResMed:
• Simpler interface, but fewer customization options.
• You can’t adjust the backup rate or breath-trigger sensitivity.
• Limited to a narrower clinical application spectrum, but incredibly easy for most patients.
Lowenstein:
• Much more tunable: trigger/cycle sensitivity, rise time, backup rate, volume targets, etc.
• Better for complex or difficult-to-treat patients like those with Cheyne-Stokes respiration or a mix of CSA/OSA.
• Bit of a learning curve for clinicians due to the extra settings.

Yep old Darth vader has done me well for over a year. For some reason my Resmed S10s for some reason just didn't cut it for me in early january last year - I figure my body must have just not likes the resmed algos anymore. Old Darth vader sorted that crap out straight away. Only issue is that some mornings I wake up with an intent to jump on a tank and roll into poland.

That's interesting, can you download the app? The Prisma Smart should work if you can download it as the bluetooth is internal on that device. If you have Darth (The PrismaLine series) then the bluetooth module/modem is external - It bolts on to the side

@CPAPReviews I contacted Lowenstein and asked why I couldn't get their app to work in the USA. They told me sorry it's a no go in the US.
They said this app isn't available on Google Play in your region.

I have heard of it. It actually is my third bed partner. 😉 and it's silent. Controllable by my phone and it sends my "anonimised" sleeping data over it's own cellular connection to my doctor. Combined with a philips dreamwisp mask (hose connection on top of my head) it works like a charm. I'm sleeping well. It's a well thought out machine.

@Nick i've ordered the Prisma 20A here in the UK on your research and recommendation. I got my diagnosis from a private test about 12 days ago. The only problem is being from the UK i've told the NHS but it's along wait to get an actual care provider hence buying my own machine. What settings do you recommend starting with? I'll be buying Sleep HQ Pro and I expect lots of tweaking and changing i'm just not sure where to begin - I had an AHI of 25 and AHIc of 10 on my test, should I start with between 6 and 12 on the Prisma 20A?
Hey mate, i highly recommend these devices for a number of reasons. The build quality is elite, the durability is elite and the algorithm is elite. It's a good choice. i'd probably just start at 4-20cm dude and then just play around with the SoftPAP pressure relief and see how you feel. Of course i'd love you to become a pro member but you're welcome to use it for free also. Good luck man, i'm in the pro community every week so i'll keep an eye out for you. Cheers
@@CPAPReviews Thanks Nicko! It arrived today - however I don't have a mask yet. I was looking around at something in the UK I could get quick but I ordered an AirTouch N30i from Australia of all places, small world eh! It's not out in Europe or the UK yet and I really like the look of it.
I'm definitely signing up to pro and hopefully get this dialled in. So if i'm right in thinking on APAP you put it to the maximum of 20cm but it'll only go there if it feels you're not getting results lower right? So it doesn't make much sense to cap it as such as it'll only go there if you need it?
Also did I see you say in another comment somewhere that we can somehow link the USB port an iPad or a phone and and upload direct to SleepHQ to save moving the SD card?
